Prescription request

If you require a repeat prescription, please contact the practice by completing the form below.

For the avoidance of clinical risk, you will need to state which medication and dosage of medication you require. We will not be able to issue your prescription if you do not clearly state the medication you require.

Prescriptions can only be issued for patients who are in active management, otherwise if not in treatment your general practitioner would be the person to prescribe for you.

Please note: We do not charge for prescriptions issued during appointments. At all other times there is a charge of £25 per prescription.

Please allow at least 72 working hours to arrange your prescription.

We prefer to use Pharmacierge as our direct-to-patient prescription delivery service. Pharmacierge are able to offer free delivery directly to your door, across London and the UK. Their charges for medication are in-line with High Street pharmacies, and you are able to pay via telephone or online payment.
